JOB SUMMARY

Service Coordinators maintain and coordinate a caseload of
caregivers who are providing services for Clients enrolled in Medicaid
programs. These duties include assisting with day-to-day functions as
well as answering questions related to the provision of these services, while
following company policy, Federal/State regulations, program rules, and
contract requirements.

JOB DUTIES

Assist IPs, Clients, and stakeholders through multiple communication channels (e.g. phone, email, in-person, etc.)
Demonstrate excellent customer service skills
Assist caregivers in navigating services, web portal, and EVV systems as needed
Ability to demonstrate dependability and to meet position requirements with little direct oversight
Effective written and verbal communicator
Demonstrate effective problem-solving and decision-making skills
Exhibit computer proficiency
Bilingual preferred in various locations
Some travel in region may be required up to 20%
Other duties as assigned

QUALIFICATIONS

Bachelor’s degree preferred, but not required
1 year of previous experience working with individuals with disabilities or the elderly preferred
Combination of education and experience Reliable transportation required

The incumbent typically works in an office environment and uses a computer, telephone, and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday. The employee is regularly required to sit, talk, or hear; frequently required to use repetitive hand motion, handle or feel; and to stand, walk, reach, bend or lift up to 20
pounds.
