We are looking for a Transitional Care Worker who will work in collaboration with the Senior Manager of Community Health and Wellness of the Sault Ste. Marie Branch. The incumbents will provide seniors, and their caregivers support to safely return and settle at home after a hospital stay through the Priority Assistance to Transition Home (PATH) program. They will facilitate and support client independence by providing transitional services from hospital to home, which reflect the unique needs of each client, and will be part of a program that supports their local community and improves quality of life of our clients.

In this role, you will:

  • Provide support to client during hospital discharge process and during the transition from hospital to the client’s home in accordance with established service plan.
  • Complete basic errands and home management duties as required, in accordance with established service plan, i.e., preparing nutritious meals while safely and hygienically storing and handling food and maintaining the comfort and safety of the client’s immediate environment.
  • Complete risk analysis of home using standardized Risk Assessment tool.
  • Facilitate client and caregiver engagement, referrals to Community Support Services and discharge process as per program guidelines.
  • Identify and report any unsafe equipment and situations, handle and store hazardous products safely (WHMIS).
  • Provide companionship to clients within therapeutic boundaries.
  • Observe and report any concerns related to the client or their environment to the Program Coordinator.
  • Maintain confidentiality of clients, caregiver/family, and the Canadian Red Cross Society.

What we are looking for:

  • Completion of two years of post-secondary education with a concentration in social services, gerontology, system navigation, and/or personal support worker background with extensive experience, or a related field, with a minimum of three years job-related experience or an equivalent combination of education and experience.
  • Bilingualism (English and French) is an asset
  • Previous experience with elderly clients is an asset.
  • Interpersonal skills typically gained through daily living skills: dependable, punctual, ability to manage stress and interacts well with others.
  • Ability to follow care plan as identified by the appropriate funder agency.
  • Self starter and able to work in an environment with ambiguity.
  • Proven analytical and problem-solving skills, ability to be a supportive and productive team member.

Working conditions:

  • Duties are principally conducted at the residencies of the clients, located within Sault Ste. Marie.
  • The work can require irregular hours, including weekends and holidays, without notice.
  • A good range of motion is required to assist clients with an arm-assist or pushing a wheelchair.
  • This position requires a successful Canadian criminal record, reference check, valid provincial driver’s license as well as access to a reliable vehicle.
